You can take it at any time of day, with or without food, and without the water and waiting rules that apply to some other oral options. This gives patients a flexible oral option for weight management.

How Foundayo Is Taken

Foundayo is taken once daily. Swallow the tablet whole. Do not break, crush, or chew it.

Treatment starts at a low dose and increases in steps. This gradual approach gives your body time to adjust and reduces stomach-related side effects. Dr. Okoye sets your schedule and adjusts it based on how you respond and how well you tolerate each step.

The maximum dose is 17.2 mg once daily. Not every patient needs to reach the highest dose. The right dose is the one that works for you and that you tolerate well.

If you miss one dose, take it as soon as possible and do not double the next dose. If you miss seven or more doses in a row, contact the clinic. Foundayo should be restarted at a lower dose according to the prescribing instructions, then titrated again, which reduces the risk of stomach side effects.

Safety, Side Effects, and Who Should Not Take It

Common Side Effects

Most side effects are digestive, tend to appear when starting or increasing a dose, and often ease as your body adjusts. Reported side effects include nausea, constipation, diarrhea, vomiting, indigestion, abdominal pain, bloating, headache, fatigue, burping, reflux, gas, and hair loss.

Serious Risks to Know

• Pancreatitis. Severe or persistent abdominal pain, sometimes spreading to the back, with or without nausea or vomiting. Stop the medication and seek care.
• Severe stomach and bowel reactions. Foundayo is not recommended for patients with severe gastroparesis.
• Gallbladder problems, including gallstones and gallbladder inflammation.
• Kidney injury from dehydration caused by significant nausea, vomiting, or diarrhea.
• Low blood sugar, particularly when used alongside insulin or medications that increase insulin release. Those doses may need adjustment.
• Serious allergic reactions, including swelling of the face, lips, tongue, or throat.

Foundayo is not recommended for patients with severe liver impairment. Tell every member of your care team you take Foundayo before any surgery or procedure involving anesthesia or sedation, because the medication slows stomach emptying.

Birth Control: An Important Instruction

Patients who can become pregnant and who use oral contraceptives should switch to a non-oral method, or add a barrier method, for 30 days after starting Foundayo and for 30 days after each dose increase. Dr. Okoye reviews this with you directly at the visit where Foundayo is prescribed.

Pregnancy and Breastfeeding

Foundayo should not be used for weight loss during pregnancy. Weight loss offers no benefit to a pregnant patient, and the medication may cause fetal harm. If you become pregnant, contact the clinic and stop Foundayo as directed. Talk with Dr. Okoye about breastfeeding before starting or continuing treatment.

Other Medications

Foundayo is processed through the CYP3A4 pathway, and it slows stomach emptying, so it interacts with a number of common medications in specific ways. Bring a complete list of everything you take, including over-the-counter products and supplements, to your visit. Reviewing that list is a standard part of your evaluation.

• Strong CYP3A4 inhibitors. The maximum Foundayo dose is 9 mg once daily when taken with one. Strong inhibitors that also block OATP1B, such as ritonavir, are avoided.
• Strong CYP3A4 inducers. These are avoided, because they lower Foundayo levels and can make it less effective. With moderate inducers, your physician monitors effectiveness and adjusts the dose as needed.
• Simvastatin. The prescribing information specifically limits simvastatin to 20 mg once daily when used with Foundayo.
• Insulin and medications that increase insulin release. These raise the risk of low blood sugar and their doses may need to be reduced.
• Oral medications generally. Because Foundayo delays gastric emptying, it can affect how other oral medications are absorbed.
• Other GLP-1 medications. Foundayo is not used together with another GLP-1 receptor agonist. If you currently take one, tell us at your visit so any change is managed by your physician.

Insurance, Coverage, and Cost

We accept Medicaid, Medicare, and most commercial insurance plans. Self-pay options are also available. HSA, FSA, and CareCredit are accepted for eligible services.

Eli Lilly operates manufacturer programs that lower the cost for eligible patients. As of August 2026, eligible commercially insured patients may pay as little as $25 per month, manufacturer self-pay pricing is set in tiers that vary by dose, and eligible Medicare Part D patients may access reduced pricing through the Medicare GLP-1 Bridge.
