A GLP-1 You Take as a Pill

Foundayo™ (orforglipron) is a brand-name prescription medication from Eli Lilly, FDA approved on April 1, 2026 for chronic weight management. It is a GLP-1 receptor agonist, the same class of medication as Wegovy® and Zepbound®. The difference is how you take it. Foundayo is a small-molecule tablet you swallow once a day.

You can take it at any time of day, with or without food, and without the water and waiting rules that apply to some other oral options. This gives patients a flexible oral option for weight management.

At Charlotte Weight Loss & Wellness Clinic, Foundayo is reviewed during your medical visit alongside your health history, current medications, lab work, and coverage. It is one tool inside a complete physician-led program, not a prescription handed over and forgotten.

We accept Medicaid, Medicare, and most commercial insurance plans. Self-pay options are also available.

If Injections Have Been the Barrier

Needle anxiety can keep some patients from considering injectable treatment at all. Others manage injections fine and simply prefer a pill. Some travel for work and want something that does not require refrigeration or a weekly routine. Those are practical reasons, and they matter.

Foundayo removes the injection from the conversation. For patients who have delayed a conversation about treatment because of that one obstacle, an oral option is worth raising at your visit. Whether it fits your health is decided after a medical evaluation.

No Injection

A film-coated tablet you swallow whole. Nothing to mix, measure, or inject.

Flexible Timing

Morning, evening, or midday. With food or without. Your schedule decides.

Room Temperature

No refrigeration, which makes travel and daily storage simpler.

How Foundayo Is Taken

Foundayo is taken once daily. Swallow the tablet whole. Do not break, crush, or chew it.

Treatment starts at a low dose and increases in steps. This gradual approach gives your body time to adjust and reduces stomach-related side effects. Dr. Okoye sets your schedule and adjusts it based on how you respond and how well you tolerate each step.

StepDoseMinimum time before increasing
Starting dose0.8 mg once dailyAt least 30 days
Second step2.5 mg once dailyAt least 30 days
Third step5.5 mg once dailyAt least 30 days
Maintenance options9 mg, 14.5 mg, or 17.2 mg once dailySet by your physician

The maximum dose is 17.2 mg once daily. Not every patient needs to reach the highest dose. The right dose is the one that works for you and that you tolerate well.

If you miss one dose, take it as soon as possible and do not double the next dose. If you miss seven or more doses in a row, contact the clinic. Foundayo should be restarted at a lower dose according to the prescribing instructions, then titrated again, which reduces the risk of stomach side effects.

Who Foundayo Is Approved For

Foundayo is FDA approved for adults with obesity, or adults with overweight who have at least one weight-related medical condition, used together with a reduced-calorie diet and increased physical activity, to reduce excess body weight and maintain that reduction long term.

Foundayo is approved for adults only. It is not approved for children or adolescents.

How Coverage Criteria Usually Work

Insurance plans apply their own coverage and prior-authorization rules, which are separate from the FDA indication. Plans commonly reference:

  • A body mass index (BMI) of 30 or higher, or
  • A BMI of 27 or higher with at least one weight-related condition, such as high blood pressure, type 2 diabetes, high cholesterol, or obstructive sleep apnea.

These are common payer criteria, not FDA labeling language. Your plan sets its own requirements.

This is published FDA and insurance information, not a decision about your individual care. Dr. Okoye reviews your full health history during your visit and determines with you whether Foundayo fits your plan. See what to expect at your first visit.

Safety, Side Effects, and Who Should Not Take It

Boxed Warning: Thyroid C-Cell Tumors

Medications with GLP-1 receptor agonist activity have caused thyroid C-cell tumors in rodents. Orforglipron itself did not produce these tumors in rodent studies, and it is not pharmacologically active in rats or mice. Whether GLP-1 related thyroid C-cell tumors seen in rodents apply to humans has not been determined.

Foundayo should not be used by anyone with a personal or family history of medullary thyroid carcinoma (MTC), or by anyone with multiple endocrine neoplasia syndrome type 2 (MEN 2). It should also not be used by anyone with a known serious allergic reaction to orforglipron or its ingredients.

Common Side Effects

Most side effects are digestive, tend to appear when starting or increasing a dose, and often ease as your body adjusts. Reported side effects include nausea, constipation, diarrhea, vomiting, indigestion, abdominal pain, bloating, headache, fatigue, burping, reflux, gas, and hair loss.

Serious Risks to Know

  • Pancreatitis. Severe or persistent abdominal pain, sometimes spreading to the back, with or without nausea or vomiting. Stop the medication and seek care.
  • Severe stomach and bowel reactions. Foundayo is not recommended for patients with severe gastroparesis.
  • Gallbladder problems, including gallstones and gallbladder inflammation.
  • Kidney injury from dehydration caused by significant nausea, vomiting, or diarrhea.
  • Low blood sugar, particularly when used alongside insulin or medications that increase insulin release. Those doses may need adjustment.
  • Serious allergic reactions, including swelling of the face, lips, tongue, or throat.

Foundayo is not recommended for patients with severe liver impairment. Tell every member of your care team you take Foundayo before any surgery or procedure involving anesthesia or sedation, because the medication slows stomach emptying.

Birth Control: An Important Instruction

Patients who can become pregnant and who use oral contraceptives should switch to a non-oral method, or add a barrier method, for 30 days after starting Foundayo and for 30 days after each dose increase. Dr. Okoye reviews this with you directly at the visit where Foundayo is prescribed.

Pregnancy and Breastfeeding

Foundayo should not be used for weight loss during pregnancy. Weight loss offers no benefit to a pregnant patient, and the medication may cause fetal harm. If you become pregnant, contact the clinic and stop Foundayo as directed. Talk with Dr. Okoye about breastfeeding before starting or continuing treatment.

Other Medications

Foundayo is processed through the CYP3A4 pathway, and it slows stomach emptying, so it interacts with a number of common medications in specific ways. Bring a complete list of everything you take, including over-the-counter products and supplements, to your visit. Reviewing that list is a standard part of your evaluation.

  • Strong CYP3A4 inhibitors. The maximum Foundayo dose is 9 mg once daily when taken with one. Strong inhibitors that also block OATP1B, such as ritonavir, are avoided.
  • Strong CYP3A4 inducers. These are avoided, because they lower Foundayo levels and can make it less effective. With moderate inducers, your physician monitors effectiveness and adjusts the dose as needed.
  • Simvastatin. The prescribing information specifically limits simvastatin to 20 mg once daily when used with Foundayo.
  • Insulin and medications that increase insulin release. These raise the risk of low blood sugar and their doses may need to be reduced.
  • Oral medications generally. Because Foundayo delays gastric emptying, it can affect how other oral medications are absorbed.
  • Other GLP-1 medications. Foundayo is not used together with another GLP-1 receptor agonist. If you currently take one, tell us at your visit so any change is managed by your physician.

Foundayo Compared With Injectable GLP-1 Options

There is no single best weight loss medication. The right choice depends on your health, your coverage, your tolerance, and your preferences. Here is how the options differ in practical terms.

Insurance, Coverage, and Cost

We accept Medicaid, Medicare, and most commercial insurance plans. Self-pay options are also available. HSA, FSA, and CareCredit are accepted for eligible services.

Coverage for your office visit and coverage for your medication are two separate benefits under most plans. Our team helps you understand both. Where a plan requires prior authorization, we review the requirements, submit the request, and supply the supporting clinical documentation. Final approval decisions belong to your insurance company.

If Your Plan Does Not Cover Foundayo

Eli Lilly operates manufacturer programs that lower the cost for eligible patients. As of August 2026, eligible commercially insured patients may pay as little as $25 per month, manufacturer self-pay pricing is set in tiers that vary by dose, and eligible Medicare Part D patients may access reduced pricing through the Medicare GLP-1 Bridge. Manufacturer programs carry their own eligibility rules, enrollment steps, refill timing conditions, and expiration dates. Current amounts are published by the manufacturer at foundayo.lilly.com.

Charlotte Weight Loss & Wellness Clinic does not sell medications and does not set these prices. Program eligibility, medication availability, and pricing can change. Medication costs are paid directly to the manufacturer's pharmacy or a participating pharmacy, not to the clinic. Commercial copay savings cards generally exclude patients enrolled in Medicare, Medicaid, or other government-funded insurance.

See current manufacturer prices and eligibility, our insurance coverage guide, or clinic pricing.

We prescribe only brand-name, FDA-approved medications. We do not prescribe compounded semaglutide, compounded tirzepatide, or compounded orforglipron.

Frequently Asked Questions

What is Foundayo?

Foundayo (orforglipron) is a brand-name prescription tablet from Eli Lilly, FDA approved in April 2026 for chronic weight management in adults. It is a GLP-1 receptor agonist taken once daily by mouth.

Is Foundayo the same as Ozempic or Mounjaro?

No. Ozempic and Mounjaro are FDA approved for type 2 diabetes, not for chronic weight management. Foundayo is approved for weight management and is taken as a daily tablet rather than an injection.

Do I have to take Foundayo at a certain time or on an empty stomach?

No. Foundayo can be taken at any time of day, with or without food, and without water restrictions. Swallow the tablet whole rather than breaking, crushing, or chewing it.

How quickly does the dose increase?

Treatment starts at 0.8 mg once daily. Each increase happens after a minimum of 30 days on the current dose. Dr. Okoye decides the pace based on your response and tolerance, up to a maximum of 17.2 mg once daily.

What are the most common side effects?

Digestive symptoms are the most common, including nausea, constipation, diarrhea, and indigestion. They usually appear when starting or increasing a dose and often ease with time. Contact the clinic if symptoms are severe or persistent.

Who should not take Foundayo?

Foundayo should not be used by anyone with a personal or family history of medullary thyroid carcinoma, anyone with multiple endocrine neoplasia syndrome type 2, or anyone with a known serious allergic reaction to orforglipron or its ingredients. It is not recommended for patients with severe liver impairment or severe gastroparesis, and it should not be used for weight loss during pregnancy.

Can I take Foundayo if I am already on Wegovy, Ozempic, or Zepbound?

Foundayo is not used together with another GLP-1 receptor agonist. If you are currently taking one, tell us at your visit. Any change from one medication to another is planned and managed by your physician.

Does insurance cover Foundayo?

Coverage depends on your plan. Some plans cover it, some require prior authorization, and some exclude weight loss medications. Our team helps prepare and submit prior authorization requests and reviews manufacturer savings programs with you if your plan does not cover it.

Can I get Foundayo through a virtual visit?

Yes. Initial and follow-up visits are available in person or virtually for patients located in North Carolina and South Carolina.

Do you prescribe compounded orforglipron?

No. We prescribe only brand-name, FDA-approved medications filled through licensed pharmacies.

How do I get started?

Schedule an appointment online or call (980) 306-2060. You can also complete the screening form as a first step. Completing the form does not determine eligibility or guarantee treatment. Final recommendations are made after a medical evaluation.
